Lockers will be issued only by request of the student. Students must use the request form embedded below. The attendance office will not issue a locker to students at their office! Students will receive their locker number/combination via email. If a student encounters a problem, he/she should see the attendance office.
Note: In order to fill out the form, the student should be logged in to his/her school Google account in order to fill out the form. Students in the Class of 2021 who did not attend summer school will not have their Google accounts until after school starts. We apologize for this inconvenience.
